Creating my CareMap
Participants will be guided through the process of creating a CareMap, which focuseson the care provider’s support system—what is working well and what and where support is needed. They will understand how the tool works, and create their own CareMap.
Objectives
1. Promotores will be able to describe the benefits of using a CareMap as a tool for family caregivers.
2. Promotores will create their own CareMap.
3. Promotores will learn to use CareMaps as self-guided tools with their family, clients, and their community.
